We are currently seeking a highly skilled Rehabilitation Specialist to join our team at Life Care Center of Kona. As a key member of our interdisciplinary team, you will play a vital role in developing and implementing comprehensive treatment plans for patients recovering from illness or injury.
In this role, you will be responsible for:
- Conducting thorough assessments to identify patient needs and goals
- Developing and implementing individualized treatment plans in collaboration with multidisciplinary teams
- Providing direct physical therapy services, including exercise instruction and education on proper body mechanics
- Evaluating patient progress and making adjustments to treatment plans as needed
The ideal candidate will have:
- A Doctor of Physical Therapy (DPT) degree from an accredited program
- Current licensure as a Physical Therapist in the state of Hawaii
- Experience working in a post-acute care setting preferred
- Excellent communication and interpersonal skills
