Job Summary
Life Dome is seeking a compassionate and highly motivated person. As a Community Mental Health Support Staff, you'll have the opportunity to provide HCBS and CORE services to our participants both in-person and when deemed necessary remotely. Under the leadership of the Program Director, your focus will be on fostering connections, promoting independence, and ensuring individuals receive the support they need to thrive independently as possible in their communities. From developing Individualized Service Plans to advocating for entitlements and connecting participants to vocational and social opportunities, you'll play a crucial role in transforming lives using the Psychiatric Rehabilitation model. As a Community MH Support Staff you will attend and Utilize the Psychiatric Rehabilitation Model to promote goal support using Orienting skills, Active Listening skills, Engagement skills.
Responsibilities
- Provide HCBS and CORE services for a caseload of up to 20 participants in person and remotely.
- Provide person-centered, strength-based, and trauma-informed services following all state, Medicaid, and program guidelines.
- Document all participant interactions, goal development, and service planning in collaboration with the participant.
- Submit encounter notes within 48 hours and maintain compliance with LDAS billing protocols.
- Ensure participants are met as outlined in their service plan.
- Ensure all appointments are maintained on the LDAS program calendar.
- Ensure that all required documentation is completed thoroughly and promptly.
- Provide pathways to connect participants to the cultural, social, and recreational offerings in their communities.
- Promote job readiness, and secure connections to vocational and community-oriented opportunities.
- Provide advocacy and instruction to gain appropriate entitlements and services to promote independence.
This position requires dedication to enhancing the quality of life for individuals facing mental health challenges through compassionate care and support.
Qualifications
- Proficient in applied behavior analysis and patient care
- Knowledge of HIPAA regulations and medical documentation
- Familiarity with medical terminology and records management
- Experience in crisis intervention
- Skills in addiction counseling and special education techniques
