Position Overview:
The Developmental Disability Documentation Specialist provides personalized support and services to individuals with developmental disabilities, promoting their independence, well-being, and inclusion in the community. This role involves assessing client needs, developing and implementing individualized plans, coordinating services, and ensuring compliance with regulatory standards. The Developmental Disability Specialist acts as an advocate for individuals and collaborates with families, caregivers, and service providers to achieve positive outcomes.

Key Responsibilities:

  • Individualized Support Planning:
  • Conduct assessments to identify the needs, strengths, and preferences of individuals with developmental disabilities.
  • Develop and implement individualized service plans (ISPs) that align with client goals and regulatory requirements.
  • Regularly review and update plans to ensure they remain effective and responsive to changing needs.
  • Advocacy and Empowerment:
  • Advocate for the rights and needs of individuals with developmental disabilities.
  • Support clients in developing life skills, achieving personal goals, and participating in community activities.
  • Promote self-determination and independence through person-centered approaches.
  • Coordination of Services:
  • Collaborate with families, caregivers, and other stakeholders to coordinate services and resources.
  • Connect clients with community programs, educational opportunities, vocational training, and healthcare services.
  • Monitor service delivery to ensure alignment with the ISP and address any gaps in support.
  • Training and Education:
  • Provide training and guidance to families, caregivers, and staff on best practices for supporting individuals with developmental disabilities.
  • Educate clients on self-advocacy, safety, and community resources.
  • Documentation and Compliance:
  • Maintain accurate and thorough documentation of assessments, service plans, and progress reports.
  • Ensure compliance with local, state, and federal regulations, including Medicaid waiver programs and other funding guidelines.
  • Participate in audits, reviews, and quality assurance activities as needed.
  • Crisis Intervention and Problem-Solving:
  • Address challenges or crises as they arise, utilizing appropriate de-escalation and intervention strategies.
  • Develop and implement behavior support plans in collaboration with behavioral specialists and other professionals.

Qualifications:

  • Education:

Currently in school to receive, or have a Bachelor’s degree in Social Work, Psychology, Special Education, Human Services, or a related field.

  • Experience:
  • Minimum of 2 years of experience working with individuals with developmental disabilities.
  • Familiarity with Medicaid waiver programs, DBHDS guidelines, or other state-specific disability services preferred.
  • Skills and Abilities:
  • Strong understanding of developmental disabilities, person-centered planning, and behavior support strategies.
  • Excellent communication and interpersonal skills for working with clients, families, and multidisciplinary teams.
  • Proficiency in documentation, record-keeping, and using case management software.
  • Ability to manage multiple cases and prioritize effectively.
  • Certifications:
  • CPR, First Aid, and other relevant certifications may be required.

Work Environment:

  • Typically, Hybrid. One or two days of office-based work weekly with frequent travel to community settings, or service providers.
  • Flexible schedule may be required, including occasional evenings or weekends.

Job Type: Contract

Pay: From $17.00 per hour

Expected hours: 6 – 18 per week
