What are the responsibilities and job description for the Direct Support Professional Contract Services position at Life Inc?
Position Title : Direct Support Professional Contract Services Female Staff Required
Shifts : 8 : 00AM - 2 : 00 PM Monday - Thursday
This is not set hours, this position is more like PRN, because the consumer is very independent and lives at home.
You will know in advance what days and hours you are needed.
This is a position to help supplement income and will not be consistent.
Hours may vary due to consumer having Dr. Appointments Must have reliable transportation Must have valid drivers license and insurance
Compensation : $17.50 / hour

Responsibilities
Transport the client to and from the day program.
Assist client according to his / her Person-Centered Plan.
Assist and monitor clients during meals to ensure utilization of family style dining.
Assist and monitor clients in all aspects of housekeeping responsibilities (Clothing care, house cleaning, and yard care).
Assist and monitor clients in preparing meals and nutritional supplements.
Assist and monitor clients in the administration of prescribed medication without error.
Assist and monitor clients in the implementation of prescribed diet orders and utilization of adaptive equipment.
Implement and follow all established behavior programs, data collection, and established safety procedures.
Assist and monitor clients in appropriate active treatment including training objective plans, recording data, and implementing service goals.
Participate as a member of the interdisciplinary team and to communicate appropriate client’s information as directed by supervisory staff.
Assist and monitor clients during community outings.
Assist and monitor clients during participation at school and day program activities.
Qualifications
Minimum of High School Diploma or GED
Must have own reliable transportation and insurance with proof.
NC Driver's License
21 years of age or older
