What are the responsibilities and job description for the Paraprofessional - Behavioral Health position at Life Rediscovery Center, LLC?
The position is a contract position. This position requires applicant to provide supportive mental health and/or substance abuse services in the client's home, community setting, or other natural environments.
Duties Include:
- Provide skills training and case management to promote stability and assist client towards functioning in their daily environment.
- Locate community resources and provide referrals as necessary based on client's need.
- Manage a caseload of a minimum of 10 clients by scheduling appointment's, evaluating client's progress in each session, and making recommendations to the treatment team.
- Collaborate with all assigned treatment team members to implement individualized recovery plan.
- Complete accurate documentation including, but not limited to, services rendered and monthly reports in a timely manner.
- Assistance in scheduling and the gathering of information for the assessment and reassessment process.
- Report all incidents of treatment to Clinical Supervisor and/or Client Liaison including, but not limited to, treatment non-compliance, safety, conflict of interest, boundary issues, abuse, and/or neglect.
- Ensure confidentiality and rights of individuals served are maintained at all times.
- Attend monthly agency-wide meetings as required.
- Attend staffing and staff training activities as required.
- Additional duties as necessary and requested by Clinical Staff.
Education:
- Must have a High School Diploma (experience working in Mental Health is preferred)
- Bachelors/Masters Degree in Social Science, Psychology, Sociology, Counseling (helping field) Preferred
- Sharenote Experience Preferred
