Job Summary
We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong understanding of medical documentation, patient assessment, and the ability to work independently while collaborating with a multidisciplinary team.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans based on patient needs.
- Administer medications and treatments as prescribed, including tube feeding and emergency interventions when necessary.
- Monitor patients’ vital signs, health status, and response to treatments, documenting all findings accurately.
- Provide education and support to patients and their families regarding health management, including pediatrics and trauma medicine.
- Collaborate with physicians and other healthcare professionals to coordinate care and ensure optimal patient outcomes.
- Maintain accurate medical documentation in compliance with regulatory standards.
- Assist in outpatient procedures within an ambulatory surgery center setting as needed.
- Stay informed about advancements in nursing practices related to hospital medicine, emergency medicine, and dermal fillers.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Strong knowledge of anatomy and physiology as it relates to patient care.
- Experience in pediatrics or trauma medicine is preferred but not required.
- Proficiency in medical documentation practices is essential.
- Ability to perform nursing duties independently while demonstrating excellent critical thinking skills.
- Previous experience in home health or similar settings is a plus.
- Excellent communication skills with a focus on patient education and support.
- Willingness to work flexible hours as needed for patient care.
