Summary
SUMMARY:
This role will work 4/10 hr shifts and commit to every 4th weekend.
The Emergency Department Social Worker, in collaboration with the clinical team and medical provider, provides patient and family advocacy, discharge planning coordination, and psychosocial intervention for the high risk patient presenting to the Emergency Department.
This role is responsible for discharge planning coordination to the general medical population in the ED and not the Psych ED.The Social Worker strives to promote patient and family wellness, improved care outcomes, and access to appropriate hospital and community resources among a patient population with complex health needs.
REQUIREMENTS:
- Seasoned professional knowledge; equivalent to a Master's degree; knowledge in more than one discipline
- LMSW or LCSW-C & MSW required
- Critical Incident Stress debriefing training preferred (ED Social Worker)
- MD Social Work License per level of education
