Responsibilities
- Conduct standardized comprehensive needs assessments to identify patient goals and develop personalized care plans.
- Carefully coordinate referrals and transitions of care between healthcare providers and settings.
- Foster strong relationships with patients, families, and caregivers to ensure timely access to appointments and services.
- Educate patients and their support networks on self-care techniques, medication adherence, and healthy lifestyle choices.
- Monitor and evaluate the effectiveness of care plans in collaboration with Interdisciplinary Teams (IDTs).
- Maintain accurate and detailed documentation of patient interactions, care plans, and outcomes.
