Summary
JOB SUMMARY: Through home based services, provides appropriate modeling techniques, education, advocacy, and other supportive services to assist in the prevention of child abuse and neglect for families at risk. Maintains regular and long-term (up to three years) relationships with enrolled families.
ESSENTIAL FUNCTIONS:
- Collects information related to nurturing parent- child interaction, childhood growth and development, enhancement of family functioning, health, and environmental factors to address unmet needs and psychosocial barriers.
- Provides education and support on parenting skills, child development, health literacy and medical compliance, financial stability, and safety.
- Svcs Coord/Advocacy Provides ongoing services coordination to address identified needs.
- Maintains all documentations required by Healthy Families America, funders, and hospital.
- Extends information to the public and to participants within the community and in the home.
REQUIREMENTS: Experience: Required: * 1-3 years Education: Preferred: * High School / GED
