Job Summary

As a Registered Nurse, you will be responsible for assessing patients' needs, developing and implementing personalized care plans, and providing emotional support to patients and their loved ones during this challenging time.
Responsibilities
- Assess patients' physical, emotional, and spiritual needs and develop comprehensive care plans
- Provide high-quality nursing care to patients in their homes, focusing on pain management, symptom control, and overall well-being
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication
- Offer emotional support and education to patients and their families, empowering them to make informed decisions about their care
Requirements
- Current RN licensure in Maryland and Pennsylvania
- Minimum 2 years of medical-surgical experience, with preference given to those with home health experience
- Expertise in performing health assessments and developing care plans
- CHPN certification or willingness to obtain within 4 years of hire
