Hospice Interdisciplinary Team Member
We are seeking a skilled Hospice Interdisciplinary Team Member to join our team at LifeBridge Health. The ideal candidate will have a strong background in clinical pastoral education and excellent communication and interpersonal skills.
Responsibilities
- Collaborate with the Interdisciplinary Team to develop a comprehensive plan of care
- Provide spiritual care to patients, family members, facility staff, and hospice colleagues
- Maintain accurate documentation records
Qualifications
- Master's Degree in Divinity or Pastoral Counseling
- Four or more units of clinical pastoral education
- One year of chaplaincy experience in a healthcare setting
- Ordination by a recognized body, church, or denomination
