In this strategic role, you will be responsible for managing a panel of patients with uncontrolled chronic conditions and/or rising risk indicators. By applying your expertise in population health management, you will identify opportunities for intervention, adjust patient assignments accordingly, and foster collaboration with healthcare providers to optimize care delivery.
Your responsibilities will include conducting comprehensive needs assessments, developing and implementing care plans, and facilitating effective transitions between healthcare settings. To succeed in this position, you should possess a Bachelor's degree in Nursing, current licensure in Maryland or a compact state, and 3-5 years of acute care experience. Strong analytical, data management, and computer skills are also required.
