JOB SUMMARY:
- Under general supervision and according to established policies and procedures, provide remote Blood Pressure (BP) monitoring for all patients enrolled in the Blip Care program. Communicates significant blood pressure changes and patient concerns to appropriate physician. Communicate with each assigned patient at least once every month and more when blood pressure is in the alert range.
JOB REQUIREMENTS:
- Licensed Practical nurse in State of Maryland
- 1 to 3 years of experience
