POSITION SUMMARY :
The Occupational Therapist Program Manager will oversee all aspects of LifeCare Alliance’s Home Repairs & Safety Program. The OT must have experience with aging in place modifications and / or home rehabilitation. Tasks will include recruiting participants, determining eligibility for services, conducting in-home assessments (pre and post), working directly with contractors, providing community education, and maintaining thorough administrative and clinical documentation. Work performed in this position will be of a complex, professional, and confidential nature. Work requires an attention to detail, flexibility, and the ability to handle multiple functions / tasks at one time. The position requires a willingness to travel in urban, suburban, and rural areas with visits in and around program participants’ homes. A positive and cooperative image is crucial for this position; work performed is under the general supervision of the Help at Home Director and in cooperation with other LifeCare Alliance program staff. This position will require a high degree of independence in the field, self-discipline, and self-direction to be successful.
This is a three-year grant-funded position. Employment in this role is guaranteed only for the duration of the grant.
PRINCIPLE RESPONSIBILITIES / ESSENTIAL FUNCTIONS :
- Perform program participant recruitment, intake, phone screenings, and in-home visits.
- Maintain thorough administrative and clinical documentation.
- Administer services via a variety of funding sources and demonstrate understanding of the requirements.
- Collaborate with general contractors.
- Demonstrate an understanding of and ability to apply home modification ideas and durable medical equipment (DME) for fall prevention.
- Perform client and family education.
- Participate in the design of comprehensive interventions targeting created to reduce fall risks.
- Assist with DME inventory and DME distribution to clients and family.
- Maintain a thorough understanding of all LifeCare Alliance programs / services, making appropriate participant referrals internally and externally, as needed.
- Complete other duties as assigned.
WORK ENVIRONMENT :
QUALIFICATIONS :
Must be a current licensed Occupational Therapist able to practice in the state of Ohio. Must have experience working in housing rehabilitation, aging in place, or other work related to home modifications. Employee must be detail-oriented and able to work in a fast-paced, deadline-driven, noisy environment. Must have a reliable vehicle, valid Ohio Driver’s License, acceptable driving record, and adequate vehicle insurance.

Knowledge of laws, regulations and guidelines pertaining to community health administration.
