This position requires 50-75% travel to Texas, Florida and Las Vegas. Must be a Registered Nurse with prior Home Health Experience.
Clinical Services Preceptor
General Summary :
To consistently strive for clinical excellence through teaching, supporting, counseling, coaching, evaluating, and serving as a role model for new and existing Registered Nurses, Licensed Vocational / Practical Nurses, and Home Health Aides. Assists in planning, presenting, evaluating, and revising staff development activities that comply with established policies, procedures, processes, federal conditions of participation, state licensure regulations, and accrediting body standards. Ensures clinical integrity through timely and accurate documentation reflective of the patient’s plan of care and interventions provided. Concurrently, participates in Clinical Manager training to perform the role in assigned agencies when need dictates.
Essential Functions / Skills :
I nstructs, counsels, and monitors the work of orientees in the aspects of home health care relative to the essential functions of the respective job; educates orientees concerning policies, procedures, and processes.Collaborates with DOCS / Clinical Manager / Administrator in planning and structuring an established orientation program to meet the individual orientation ee's needs; plans daily assignments for orientationeers.Performs ongoing assessments and regular formal evaluations of staff during the orientation period; documents all observations and discusses progress and any revisions to the orientation plan with the appropriate clinical supervisor.Performs in-home visits with orienteers to observe skills, competency, readiness, and remedial educational needs and reviews documentation for completeness.Utilizing internal and external resources, coordinates additional training / educational / credentialing offerings commensurate with agency needs, i.e., wound care, venipuncture, CPR classes.Serves as a role model, providing leadership and demonstrating professionalism.Serves as a resource by guiding new and current team members to appropriate resource materials and clinical supervision when appropriate.Guides and assists new team members into integrating company culture and environment.Nurtures, supports, and guides new and current team members through training and mentoring.Monitors time management and productivity of new and current team members.Provides valuable formative and summative feedback from the training and mentoring process to appropriate clinical supervisor along with any recommendations for successful engagement and retention.Performs random sampling of clinical record reviews to identify deficient practices that could result in potential deficiencies. Participates in performance improvement activities to ensure quality patient care and outcomes.Embraces and adheres to the company’s mission, vision, and core values.Fosters a culture of integrity, safety, and compliance.Other duties as assigned.
Participate in the development of field clinicians that routinely perform OASIS assessments and of newly hired clinicians that will be responsible for OASIS assessments.Coordinate with assigned offices to review OASIS / Plans of Care as they are completed.Review assessments based on packets available in que.Provide organization-wide direction and education for clinical staff on professional documentation.Initiate workflow improvements and standardization to increase the efficiency and accuracy of documentation for clinicians.Assist in preparing and presenting clinical education programs as directed by and in concert with the Director of Education and Quality Improvement.Possess knowledge of Medicare and Medicaid guidelines essential to Home Health CoveragePossess good organizational and time management skills.Possess good problem-solving skills.Possess strong verbal / written communication and interpersonal skills.Aptitude for thoroughness and accuracy.Initiative and the ability to work independently.Flexible, dependable, and self-directed.Computer, internet access, and email capabilities from home or other off-site locations.This description is a general statement of essential functions that are required to be performed regularly and continuously. Requirements : Education :
Currently licensed Registered Nurse, BSN preferredExperience :
A minimum of two (2) years of recent Medicare home health experience, preferably in a supervisory position possessing competency in OASIS assessmentsWorking knowledge of home health care admission criteria, federal conditions of participation, state licensure regulations, and accrediting body standards (where applicable)Must have the clinical knowledge and critical thinking ability to effectively plan and provide coordination of patient care consistent with standards and regulationsMust have exceptional interpersonal and customer service skills; must be able to effectively solve unique problems as they arise or identify when to consult othersA minimum of one (1) year experience with orienting / supervising new nursing staffReliable transportation and auto liability insurance.A current driver’s license in good standing. It is the employee's responsibility to renew their driver’s license before it expires in order to continue employment.Current and unencumbered professional license as an RN in any / all applicable states.Physical Requirements :
Prolonged sitting is required in an office environment.Ability to always handle stressful situations calmly and courteously.It requires working under some stressful conditions to meet deadlines and agency needs.Work requires considerable sitting and close-up vision work.Continually communicating with staff and other health care professionals utilizing email and telephone systemsMust be able to articulate professionally.The typical weight lifted is five pounds; the maximum weight lifted is ten pounds.Will require travel to / from home health locations for training purposes
