Provide oversight of all patient care services and personnel including making patient and personal assignments, coordinating patient care, coordinating referrals, assuring that patient needs are continually assessed, and assuring the development, implementation, and updates of the individualized plan of care. RN coordinator will need to communicate with team frequently regarding, assignments, performance, and expectations.

Weekends: On call rotations

Pay: $37.06-$55.59

- Provide direct client care
- Complete chart & compliance audits
- Complete work ques in EPIC
- Determination of referral
- Lead and prepare Quality Assurance & Performance Improvement meetings
- Keep current with regulations, policies & procedures. Update P&P as needed.
