Highlights of your role:
To provide quality patient care with the collaboration of physicians and registered nurses. The LPN is responsible to assess and document findings in the patient's chart. They will collaborate assessment, planning, implementation, and evaluation of patient care needs on an individual basis with the RN charge nurse. Responsible for communication and documentation involving the patient, significant others, and healthcare providers to coordinate optimal care.

*Pay Range:$21.23-$29.73
*It is not typical for an individual to be hired at or near the top of the range for their role. Compensation decisions are dependent on the facts and circumstances of each case and on several factors including relevant work experience, education, certification & licensure, and internal equity. What you'll get to do in this role:
Conduct a focused assessment of the health status of an individual patient through the collection and comparison of data to normal findings and the individual patient's current health status, and report changes and responses to interventions in an ongoing manner to a registered nurse or the appropriate licensed health care provider for delegated or assigned tasks or activities.
Confer with patients and/or family members to gather pertinent information including history, medications, etc., and documenting the information in the patient's chart.
Determine and implement appropriate interventions within a nursing plan of care and implement interventions that are delegated, ordered, or prescribed by a licensed health care provider
Communicate relevant patient data and changes to charge nurse/RN.
Function effectively as a professional team member by following established standards of practice through the patient care plan, report, rounds, team conference and through personal example.
Promote a safe and therapeutic environment while advocating for the best interests of individual patients
Accountability for the quality of care delivered, recognizing the limits of knowledge and experience; addressing situations beyond the nurse's competency; and performing to the level of education, knowledge, and skill ordinarily expected of an individual who has completed an approved practical nursing education program
Obtain trauma training that meets objectives specified by the Minnesota Statewide Trauma System for Level 4 Trauma Hospitals
You're an excellent fit, if you have:
current active MN LPN license
acute care experience, preferred
current BLS certification, preferred
excellent communication and interpersonal skills
safe medication administration practices
ability to care for a diverse population of patients with courtesy and respect
good teamwork and organizational skills
good decision making and time management skills
excellent clinical skills
