The Social Service Designee, will assist in the social services department of a long-term care facility. While they may not hold a social work degree, they are trained to perform essential tasks that support residents’ social and emotional needs. The designee will work under the supervision of a licensed social worker, who oversees the care plans and interventions that help enhance the quality of life for residents.
Key Responsibilities
Resident Advocacy: The Designee will advocate for residents’ rights and well-being.
Listen to residents’ concerns, preferences, and needs, and work to ensure that these are communicated to the care team. They often advocate for specific care preferences or accommodations under the guidance of the supervising social worker.
Emotional Support:
Provide emotional support to residents, helping them cope with feelings of loneliness, anxiety, or depression.
More complex emotional needs are referred to the licensed social worker or other mental health professionals.
Care Planning and Coordination:
Assist in creating and updating residents’ care plans, working closely with the licensed social worker.
Collaborate with nursing staff, therapists, and other healthcare professionals to ensure that each resident’s care plan reflects their social and emotional needs.
Coordinate additional services like counseling or therapy as needed.
Family Communication:
Maintain open communication with residents’ families.
Provide updates on the resident’s well-being and answer questions.
Works with the licensed social worker to address more complex family concerns. The Designee.
Help families understand the care process and offer support during challenging times.
Assistance with Transitions:
Provide support during the admission process and help residents and families adjust to the new environment. The licensed social worker will oversee these transitions, ensuring that they align with each resident’s care plan and emotional needs.
Resource Management:
Have knowledge about community resources and services that may benefit residents.
Provide information and referrals for services like transportation, financial assistance, or support groups.
