LifeConnections is a community mental health care provider with adult residential programs all across Massachusetts, Maine, New Hampshire and Rhode Island. We primarily serve adults with developmental disabilities and complex mental health conditions.
Position: RN Nurse Coordinator (HYBRID REMOTE!)
Program: Group Homes for Adults with Developmental Disabilities
Requirements: Current Massachusetts RN License & 2 Years of RN Experience
Salary: Up to $75K/year, Excellent benefits, laptop, cellphone & hybrid remote schedule!
Schedule: Service hours are flexible but mostly Mon-Fri. The position requires onsite presence in all the homes weekly but also provides time for remote work and is flexible as approved by the supervisor.
Summary: LifeConnections is looking for a Full Time RN Nurse Coordinator to become part of our dynamic Nursing Team. This position is for 40 hours per week, serving individuals with Intellectual / Developmental Disabilities (I/DD) living in Group Homes in North Central Massachusetts (Leominster, Fitchburg, Winchendon & Westminster).
The ideal candidate will have two years of nursing experience (any specialty), be licensed in Mass, is a team player, has MAP experience or is willing to learn (training provided). No previous experience serving adults with intellectual / developmental disabilities (I/DD) is required. Teach, consult and advocate to help individuals with intellectual/developmental disabilities reach their full potential.
Responsibilities:
- The desire to work in a community setting with individuals who have medical needs, developmental disabilities, and or mental health issues
- Oversee administration of medications and treatments following the Department of Public Health’s Medication Administration Program (MAP) policies and other funder regulations
- Complete quarterly medication audits, or as requested
- Follow medical protocols, treatment plans and medication treatment plans
- Communicate health changes in a timely manner
- Utilize Positive Behavior Supports (PBS) practices to create a thoughtful and supportive work environment by ensuring activities, plans and routines reflect the preferences, needs and goals of individuals
- Ensure skilled care is provided in a way that promotes growth, dignity, and independence
- Provide physical support to individuals who have physical challenges as needed
- Able to work with a direct care and supervisory team to ensure the health status of individuals
- Provide oversight of meals ensuring food and meals are prepared properly. Train staff on meal preparations and specialized feeding protocols as needed
- Provide training and oversight with the maintenance of medication books, house logbooks, medical tracking sheets to ensure program compliance with all regulations
- Ensure staff is ordering medications, medical and safety supplies
- Collaborate in discharge planning and follow discharge requirements for hospitalized individuals
- Train both staff and individuals-served in relevant areas (health, hygiene, safety, diet, etc.)
- Attend meetings as requested by the Regional or Company Director
- Ensure safety standards and infection control protocols are followed
- Perform other related duties as needed or as assigned by supervisor
- Participate in on call rotation for 7 days every 3 weeks (telephone triage only!)
