RN Case Manager – Discharge Planning (Acute Care)

Location: Jackson, MS 39216
Schedule: Full-time, Monday to Friday, 8-hour shifts (8:00 AM – 4:30 PM)
Key Responsibilities:

• Manage 15 to 20 patient discharges daily across adult inpatient units
• Collaborate with social workers, respiratory therapists, and pharmacy teams as needed
• Utilize EPIC EMR for documentation (experience required)
• Coordinate patient placements, including complex cases, utilizing UMMC’s contracts with local facilities

Requirements:

• Active RN license.
• Minimum 2 years of hospital-based discharge planning experience in an acute care setting.
• At least 6 months of experience in a Level 1 trauma center or large academic medical facility
• BLS Certification
• Experience with EPIC charting system

Preferred but Not Required:

• CCM Certification
• BSN (ADN candidates will be considered)
• ER discharge planning experience

Job Type: Contract

Pay: $2,404.00 - $2,606.00 per week
