Job Title: Home Health Nurse (RN)
Location: Lewes, DE 19958
Unit Type: Home Health
Shift Type: Days (8-hour shifts, Monday through Friday)
Guaranteed Hours: 40 hours per week
Key Responsibilities:
- Provide skilled nursing care and comprehensive patient assessments.
- Offer teaching, supervision, and counseling for both patients and caregivers.
- Collaborate with the multidisciplinary team to ensure optimal patient care.
- Utilize critical thinking and problem-solving skills to meet patient needs.
- Monitor and document patient progress to ensure achievement of health goals.
- Complete Oasis documentation as required.
- Administer treatments such as wound care, IVs, wound vacs, and others as directed by the physician.
- Handle specialized tasks like Pleurex drains, phlebotomy, tracheostomy care, PEG tubes, and ostomies.
- Utilize Telehealth technologies for patient monitoring and care.
Required Qualifications:
- 2 years of Home Health experience preferred.
- Registered Nurse (RN) license required.
- BLS certification required.

Pay: $49.12 - $64.85 per hour
