Social Work Case Manager – Acute Care
Location: Jackson, MS
Shift: Monday to Friday, 8:00 AM - 4:30 PM
Guaranteed Hours: 40 hours per week
Responsibilities
- Manage 15-20 cases per day, ensuring smooth care transitions and discharge planning
- Collaborate with physicians, nurses, and social workers to develop comprehensive patient care plans
- Work with community facilities, assisting with complex patient placements and financial limitations
- Assess social determinants of health and provide resources for patients and families
- Utilize EPIC charting system for documentation and case tracking
- Ensure compliance with hospital protocols, state regulations, and insurance guidelines
- Consult with respiratory therapy and pharmacy teams for discharge planning as needed
- Participate in department meetings and maintain effective communication with leadership
Requirements
- Master’s in Social Work (MSW)
- Active LMSW or LCSW license in Mississippi
- Minimum of 2 years of acute care experience
- Experience in a Level 1 Trauma Center or large academic hospital required
- Proficiency in EPIC charting system
