The Psychiatric Registered Nurse in Home Health provides and directs nursing care to patients in their homes as prescribed by physicians and in accordance with applicable laws, regulations, and agency policies.
This role requires:
- Collaborating with other healthcare professionals involved in patient care, ensuring continuity and effectiveness
- Making initial and comprehensive nursing evaluations, developing care plans within state-specific guidelines, and submitting accurate documentation in collaboration with physician orders
- Conducting patient assessments at designated times and creating personalized care plans
- Evaluating patients to ensure they meet home health eligibility and medical necessity requirements defined by payer sources
For this position, recent experience (within the last five years) or completed psychiatric retraining classes or CEUs to update knowledge is essential.
To be successful, you will need:
- Current RN licensure required
- Current CPR certification necessary
- A valid driver's license, vehicle insurance, and access to reliable transportation or public transportation are essential
