Position Summary: LifeMoves is seeking a highly motivated, self-starter to join our team of Kaiser Outreach Case Managers to provide services to homeless individuals who are being discharged from Kaiser South San Francisco. Responsibilities include providing outreach and intensive case management to unsheltered homeless individuals, perform screening assessments, make appropriate referrals, connect clients to social services, and keep up-to date case files. The successful candidates will have the ability to represent LifeMoves in a diplomatic and professional manner with partners, funders, law enforcement and other city/county entities. The Kaiser Outreach Case Manager will participate in a multidisciplinary approach to engage the hardest to serve homeless in an effort to connect them to services.
Location: South San Francisco Kaiser
Position Duties:
- The Kaiser Outreach Case Manager will be in the field 70% of the time
- Coordinate with medical care team to understand clients immediate needs
- Keep care team up to date with client’s medical statuses, additional needs and any follow up care needed
- Assist in scheduling medical appointments; including transportation; and medical appointment accompaniment
- Act as the point of contact for each client, if they need it, at medical facilities
- Transport equipment and or test results on behalf of the medical care team
- Provide relevant resources, health information and linkages based on individual health care needs
- Keep up to date case notes and data
- Develops, implements and revises individual case plans and consistently monitors progress toward client goals
- Provide crisis management for clients; makes linkages for interventions as appropriate
- Attend weekly case consultation meetings and monthly training; meet regularly with supervisor to exchange pertinent information and receive supervision
- Attend monthly case consultation meetings with partner Kaiser hospital to review referrals
- Provide on-going supportive and/or case management functions in accordance with the case/treatment plan
- Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature
Qualifications
Position Qualifications
- Bachelor’s degree in social work (preferred), psychology or related field, with a minimum of one years’ case management experience
- Excellent communication and interpersonal skills; proven ability to collaborate with community partners
- Ability to maintain a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature
- Understanding of the issues faced by homeless individuals and other marginalized populations in crisis
- Availability to work evenings and weekends.
- Experience working with mentally ill and dual diagnosed clients highly desirable
- An understanding of ideas and practices that promote diversity, equity and inclusion; and a commitment to ongoing professional development in this area.
- Bi-lingual Spanish language capabilities a plus
