COMMUNITY SUPPORT SPECIALIST
LifePath is a mission-driven, collaborative, and community-focused human services agency seeking a Community Support Specialist t o join our caring and compassionate team!
The Community Support Specialist is part of an interdisciplinary team, working closely with nurses and care managers to support older adults and individuals with disabilities to thrive in a home setting. The Community Support Specialist promotes health and well-being through education and coordination of services. The Community Support Specialist conducts assessments that are member-centered, promotes member involvement to the best of the individual's physical and psychosocial abilities, and assists with care transitions, follow-up, and accessing resources. Involves travel to perform assessments throughout Hampshire and Hampden Counties, and the North Quabbin area.
This is a full-time (35 hr / wk) Mon- Friday position. Starting pay rate of $21.60 / hour with excellent benefits including generous PTO, medical / dental / vision insurance, Flex spending, EAP, 401k and more! 6% pay differential for applicable bilingual skills.

QUALIFICATIONS :
High school diploma with 1 year of relevant experience required. Bachelor's Degree preferred.
Must have reliable transportation and be able and willing to travel throughout Franklin, Hampshire, Hampden and Northern Worcester Counties.
Demonstrated experience working with older adults or individuals with disabilities, including experience navigating benefits, resources, behavioral health systems, and long-term supports within the community.
Ability to foster self-sufficiency for individuals and families using problem-solving and practical skills; familiarity with support groups and / or recovery.
Good listening skills; Excellent oral and written communication skills, and computer proficiency.
Must be able to work independently and collaboratively to follow through with projects, meet deadlines and make positive contributions to the interdisciplinary support team.
Experience in advocacy, case management, supportive counseling, and goal-oriented problem solving.
Use of personal cell phone, data plans, and downloading of company provided app required.
ESSENTIAL FUNCTIONS / RESPONSIBILITIES :
- Conduct case management assessments of referred individuals in home settings and carry out an individualized service plan.
- Provides administrative and member onboarding support to interdisciplinary team members.
- Provide skills training and assistance in areas of daily living activities and social skills. Assist in developing communication and conflict resolution skills and act as a role model for interpersonal relationships, i.e. peer, parent / child.
- Assist with individual and family advocacy needs including identifying and obtaining entitlements, which meet member's needs. Make appropriate referrals to other agencies as needed. Serve as an advocate for members to obtain other needed services. Maintain up-to-date knowledge of information about services available.
- Assist participants with identifying appropriate behavioral health and medical services and accessing services of choice. Facilitate utilization of professional providers who can assist with care plan goals.
- Collaborate with and in some cases coordinate providers to ensure adequate implementation of service plans. This will include meetings and telephone contacts. Make appropriate referrals to other community-based agencies.
- Provide support to members and caregivers to maintain placement stability and to assist during periods of transition.
- Provide assistance with developing and maintaining a "natural support network" of family friends, and significant others in the participant's community.
- Actively participate with the interdisciplinary team and provide consistent communication with member, support network and community partners who are involved in member's care plan. Attend meetings as required.
- Maintain accurate and timely documentation in accordance with agency policies and procedures. Maintain statistics and reporting as required by the program.
- Comply with all applicable agency policies and procedures, including clinical and administrative protocols.
