What are the responsibilities and job description for the Population Health Specialist position at LifePoint Corporate?
Position Summary:
The Population Health Specialist serves as an integral member of the health care team that support and care for patients attributed to the Clinically Integrated network. The Population Health specialist will work in the ambulatory setting, specifically within primary care clinics, and is responsible for ensuring that best practices for Annual Wellness Visits, patient care gap closures, social determinants of health screenings, disease-specific patient education, and clinical quality improvements are followed. The Population Health Specialist will serve as a patient advocate by identifying barriers and navigating their health complexities to facilitate optimal care.
Minimum Qualifications:
- Bachelor degree preferred; relevant experience considered
- Previous clinical experience in a medical office, physician office or hospital required. MA or LPN preferred.
- Previous utilization review, performance improvement and case management experience desired.
- Strong computer skills with Microsoft office, electronic medical records software and other database software
Essential Job Functions:
- Documents thoroughly, following regulatory and hospital requirements.
- Performs telephone triage visits and coordinates care
- Coordinates patients Transition of Care from hospital to home and/or hospital to SNF.
- Provides group teaching for chronic disease management.
- Responsible for ensuring gaps-in-care are completed and reported.
