Position Summary
The pharmacist is a part of the healthcare delivery team and is responsible for the use of medications throughout the hospital.
Minimum Qualifications
- Graduate of an accredited School of Pharmacy
- Maintains current pharmacist license in Oregon
- Possess computer skills for order entry, word processing, clinical interaction
- Experience with drug use evaluations/monitoring, clinical interventions
- Effective communication skills with allied health care practitioners
- Experience in sterile products preparation, including TPN, Chemotherapy
- ACLS and PALS certifications
- BLS and ACLS within three months of hire and maintain as current
Essential Job Functional
- Modifies medication dosages as determined by age and weight, renal function
- Correctly interprets/documents orders and intervenes on problem medication orders/drug interactions, ADRs, DUEs, errors
- Maintains accountability within controlled drug inventory
- Performs aseptic preparation of sterile products
- Participates in clinical pharmacy initiatives, patient teaching, code support, effective use of dosing protocols, tracking of clinical interventions
Knowledge, Skills, and Abilities
- Demonstrate sound judgment, patience and maintain a professional demeanor at all times
- Ability to work in a busy and stressful environment
- Organizational skills and the ability to prioritize
- Computer skills: Word, Excel, Outlook, Electronic medical records software
- Strong interpersonal verbal and written communication skills
- Creativity, problem analysis and decision making
- Ability to work varied shifts
