What are the responsibilities and job description for the Community Habilitation Coordinator position at Lifes WORC?
- This shift is 40 hours per week (Mon-Fri 9am-5pm) some flexibility needed
- The pay rate for this position is $24.00 hourly
QUALIFICATIONS:
- Bachelor’s degree in a related field preferred
- Two year's supervisory experience in the field
- Excellent verbal and written communication skills
- Computer literacy and a willingness to learn and utilize new electronic systems
- Valid Driver’s License required and car for travel
- Ability to work a flexible schedule including weekends as required by program.
- Must have experience with OPWDD
RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:
- Oversee and supervise the day-to-day operations as it pertains to individuals, staff and work locations
- Participate in the selection and evaluation of staff, including completion of performance evaluations
- Work collaboratively and maintain positive relationships with Care Managers, people supported, families, advocates, outside agencies, and other providers to develop staff action plans.
- Collaborate with Admissions to ensure screenings, and assessments are coordinated in a timely manner
- Provide training and support for everyone in accordance with their Staff Action Plans
- Provide direct supervision to the Community Habilitation Support Staff to include consistent support, coaching and training, retention strategies, staff meetings and 1:1 supervisory support meetings as needed
- Conduct and document monthly quality assurance phone calls to families and participants.
- Conduct in field or on-site observations of staff, quarterly, regarding the implementation of person centered supports
- Review and approve Medisked daily, ensuring support staff is in regulatory compliance with OPWDD’s billing standards.
- Maintain and update all the Community Habilitation documents according to OPWDD regulations on an ongoing basis. Ensure Individuals’ documents are always audit ready
- Maintain and update required OPWDD documentation into Medisked, Choices, and other applicable platforms.
- Participate in all audits, including, OPWDD, DOH, OMIG, fiscal and internal QA
- Maintain all required OPWDD and agency training certifications including CPR/FA.
- Ensure incident reporting procedures as per regulations and agency policies
- Complete payroll and billing for Community Habilitation Staff & Individuals Medisked and Workforce Management.
- Ability to work up to 20 hours in the field as required by department needs and travel to various work locations (i.e. family homes, day programs, local businesses)
- On call 7 days per week, after business hours and on weekends to support staff and families.
- Work in conjunction with Recruitment, Training and HR to onboard new team members timely.
