Position: Senior Choice Transportation Coordinator

Reports to: Senior Choice Supervisor

Salary: $20.01/hr

JOB SUMMARY:

Lead Transportation Coordinator is responsible for daily scheduling and communicating with clients and staff. To maintain up-to-date transportation database and is responsible for documentation and timely follow-up. The Lead Transportation Coordinator will also be a back-up for Senior Choice caseworker.

RESPONSIBILITIES:

• Lead Transportation Coordinator, responsible for scheduling, communications with clients, documentation and follow up.
• Back up Transportation Driver as needed
• Coordinate Transportation trips with local community sites and Senior buildings
• Accurately complete and process referrals for clients to other agencies and private individuals.
• Provide accurate Information and Assistance for walk-in or telephone consumers.
• Stay up to date and informed on available services and how to access those services for consumers.
• Accurately and timely completion of various forms and applications for consumers with information provided by the consumer.
• Perform home visits for the purpose of assessing consumer needs, identifying available assistance and completing necessary forms, phone calls, etc. to assist the consumer in accessing needed services.
• Inform and assist clients in gaining access to available benefits and services including acting as representative at appropriate agencies with consumer consent.
• Provide timely, accurate and ongoing follow-up with consumers to insure their needs continue to be met.
• Work with staff, other agencies and private individuals to find hard-to-reach clients.
• Maintain timely and accurate required documentation of service given to clients.
• Assess consumer’s need for services not specifically funded and submit timely and accurate requests for Gap Filling services.
• Participate in activities and events to promote agency services.
• Participate in Information and Assistance activities, which may include giving presentations at various locations on benefits and services.
• Attend training as required by Supervisor or Executive Director.
• Represent the agency professionally a manner, speech and dress.
• Drive the transportation van if and when needed.
• All other duties depending on agency needs as assigned by the Supervisor, Director and/or Lifescape Administration.

ESSENTIAL FUNCTIONS:

• Must be able to read, write, and understand written and verbal instructions.
• Must be able to operate fax, copy machine, basic office machines and multiple telephone lines.
• Must be computer literate.
• Must be able to effectively and patiently communicate with consumers, caregivers, staff and visitors.
• Must be able to interpret consumers or caregivers needs effectively.
• Must be able to concisely and accurately convey those needs to the appropriate agencies.
• Must be able to interpret a variety of instructions furnished in written or oral form from government, benefit and funding organizations.
• Must be able to travel to and safely enter homes and other locations to perform job-related duties outside of the office environment.
• Must be able to work independently.
• Must be able to obtain a jo5.

MINIMUM REQUIREMENTS:

• Must be 21 years of age.
• Bachelor’s degree or equivalent experience preferred.
• Must have two years direct service experience and/or equivalent training.
• Must have valid IL driver’s license, clean driving record and own transportation with current insurance coverage. Not have been convicted within 3 years of the date of application of any of the following offenses involving the operation of a motor vehicle: Driving Under the Influence of alcohol and/or drugs (DUI), Reckless Driving, Leaving the Scene of an accident involving death or personal injury, Drag Racing, Manslaughter, Reckless Homicide, Reckless Conduct
• Must be able to work and communicate effectively with older persons, persons with sight and hearing impairments, and persons with a variety of economic, social and cultural backgrounds with an emphasis on seniors.
• Must be able to perform essential functions of position.

Job Type: Full-time

Pay: $20.01 per hour

• 8 hour shift
