Title: Targeted Case Manager - Adults
Worksite: Barren Service Center
County: Barren
Hours Status: Full-Time, M-F, 8a-5p
Pay Range: $15.87 hourly
Brief Job Description:
Advocate and provide assistance to adult clients experiencing severe mental illness and/or substance use disorders.
Responsibility includes accessing community resources, build professional relationships with community agencies, provide crisis assistance, maintain contact and monthly supervision, etc.
Qualifications:
- Bachelor’s degree in human services (Psychology, Social Work, Sociology, Family studies, Counseling, Public Health, Special Education, Education.)
- One-year post-graduation minimum experience working with SMI adults; or one year of case management experience; or 6 months if possesses master’s degree in approved field.
- Valid driver’s license and current automobile insurance.
