Brief Job Description :
The Clinical Care Manager serves as the point of first contact for service entry and assumes responsibility for ensuring the smooth and orderly coordination of multidisciplinary services to adults with severe and persistent mental illness and children with severe emotional disturbance. Provide clinical direction to community support staff in accordance with the person-centered recovery plan. This involves directing the activities of targeted case managers, peer support specialists, and behavioral health specialists.
Qualifications :
Masters degree in social work, mental health counseling, marriage and family, or psychology with certification or licensure already in process or approved by the state.
