Position Summary:
Provide nursing services to program participants, coordinate program participants' services with hospitals, outside providers and the Agency. In conjunction with the QMRP, ensure that all medical recommendations are carried out and brought to a resolution. Participate and contribute in the interdisciplinary team, maintain appropriate medical records, and cooperate with the overall policies and objectives of the Agency.
Job Duties and Responsibilities Include the Following :
- Evaluate residents medical needs and health status on a continuous basis, develop medical histories, nursing care assessments, nursing care plans, implement appropriate protective and preventive measures in regards to communicable diseases and maintain complete, accurate and up to date medical records.
- Insure residents' prescriptions are filled/refilled, physician's medical orders recorded, monitor residents' medication administration sheets to assure that medications are being administered properly, report medication errors appropriately, dispose of outdated or discontinued drugs according to the policy.
- Continually monitor the effects of resident's medications for potential adverse reactions, allergies, interactions, and contraindications.
- Prepare, schedule and provide continuing in-service training for residential staff in all required and needed subject areas, certify recertify residential staff in AMAP, train residents in appropriate health and hygiene methods.
Education and/or Experience:
Must be licensed by the New York State Department of Clinical Licensure. Specialized training or one year experience working with the developmentally disabled.
Physical Demands:
Requires moderate physical effort on a regular basis, as in frequent traveling, standing, walking, bending, prolonged sitting, stooping, or reaching and/or occasional lifting.
Working Conditions:
Apartment setting or group home environment where program participants receive structure of daily routines through supervision. Occasionally stressful environment due to challenging behavior on the part of some program participants. Therefore, must be able to maintain calm in the face of verbal, physical, and emotional challenges from program participants. Exposure to individuals who may be aggressive and may carry communicable diseases.
