What are the responsibilities and job description for the LVN Field Nurse position at LifeSpring Home and Hospice Care?
LifeSpring Home And Hospice Care is seeking dedicated Part-time Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs) to join our team. The successful candidate will be responsible for assessing patient needs, developing and implementing individualized care plans, and providing nursing care in accordance with physician orders. The ideal candidate will have excellent communication skills, the ability to work independently, and a commitment to providing the highest quality of care, care while ensuring timely documentation and excellent communication.
Responsibilities:
Provide patient care in accordance with the physician's orders and state regulations.
Perform nursing assessments and treatments as ordered by the physician.
Provide patient education on health promotion and disease prevention.
Monitor vital signs and document changes in the patient's condition.
Administer medications and treatments as ordered by the physician.
Provide home health services to patients in their homes or other community settings.
Qualifications:
- Valid Registered Nurse license in the State of Texas
- Degree in Nursing
- Current CPR certification
- Exceptional documentation and communication skills
- Proficient in Oasis documentation (RN only)
- Minimum of 1 year of home health experience
- Outgoing, driven, and patient-oriented personality
- Valid Driver's License and Auto Insurance
Job Types: Part-time, PRN
Pay: $35.00 - $40.00 per hour
Expected hours: 5 – 15 per week
