JOB DESCRIPTION
GENERAL SUMMARY
The Care Transition Coordinator (CTC) is a health care professional whose duties and responsibilities are to support patients and their family as they move through each level of care.
Essential Duties and accountabilities
- Responsible for working with individual patients and family member to navigate through different care options at different moments in their recovery or care regimens.
- Makes solid judgement and adhere to all policies ensuring the protection of confidential information related to patient demographics, medical information, diagnosis codes, PHI according to both internal and Federal HIPPA regulations and guidelines.
- Ensure that patients connect with the appropriate program or facility for their level of care.
- Advocate on the patients behalf, and ensure all parties have the appropriate patient information.
- Ensures effective communication and positive working relationships with personnel from all areas of the agency, patients, family members, doctors and facility personnel to maintain coordination of interrelated activities
- Performs job in compliance with agency policies and procedures and according to accepted standards of practice
- All other duties as assigned
SPECIAL SKILLS AND ABILITIES REQUIRED
- Knowledgeable user of computer systems and application to include Microsoft Word, Excel, Power Point, Healthwise and email
- A keen eye for details
- Ability to work methodically and meet deadlines while maintaining flexibility in meeting department and individual workload demands
- Strong familiarity with patient care transition and medical records
- Excellent interpersonal communication skills
- Organization and time management skills
- Excellent verbal and written communication
- Medical terminology
- Ability to maintain flexibility in meeting department and individual workload demands
EDUCATION
- High School diploma or GED
- LPN/LVN preferred
EXPERIENCE
- 2 years of proven experience working in health care
- Demonstrate knowledge of appropriate skills for communicating with all individuals through verbal and written tools
