GENERAL SUMMARY:
The ADvantage case Manager Supervisor will oversee the five core functions of case management: comprehensive assessment, service planning, service plan implementation, monitoring, and reassessment.
Responsibilities and Duties:
- Ensures Start of Care packets, Discharge packets and other documents required for the field staff to use are prepared and available in adequate quantities
- Implementation of Plan of Care and facilitates on-going communication to ensure that Plan of Care is updated to reflect changes in patient status, medication /equipment/supplies, interventions, goals, coordination of care with outside agencies and members as appropriate.
- Promotes the empowerment of the members/caregivers by teaching and encouraging appropriate self-care techniques.
- Prepares and maintains progress notes that accurately reflect assessment findings, changes in status, needs or level of care, interventions, teaching, and care provided, member/family response to a concern, and progress toward established goals. Paperwork is completed and submitted promptly and following agency protocol.
- Communicates with the IDT team to inform of any changes to member's POC, needs, and outcome of intervention.
- Assists to prepare for IDT meetings, conduct IDT meetings.
Experience and Education Requirements:
- High School diploma or GED
- College graduate preferred
- Licensed Practical Nursing license or Advantage certification through the state of Oklahoma
- 2 years of proven experience working in-depth with Manage Care, Medicare, Medicaid, and other regulatory programs
