Job Summary
We are seeking a highly motivated and compassionate Targeted Case Manager I to join our team of professionals. As a Targeted Case Manager I, you will play a critical role in advocating for the needs of individuals served, linking them with essential services, and monitoring their progress towards achieving their goals.
Key Responsibilities:
- Advocate in the client's best interest
- Link individuals served with needed services
- Monitor individuals served progress and provide individualized supports
- Conduct thorough assessments of consumers' home stability and develop service plans to meet their needs
- Schedule face-to-face contact with consumers and review/update service plans as required
- Involve significant others in consumers' treatment to augment or create additional supports
- Convene/attend staffing as necessary for assessing and coordinating services
- Assist consumers to develop connections to natural supports within their network
- Arrange for procurement of incidental expenses to support consumers' stability and independence
- Communicate and liaise with individuals, families, and service providers
Requirements:
- Bachelor's degree with major coursework in human services field required or equivalent experience
- Minimum one year of prior experience working with individuals with serious mental illness
- Valid FL Driver's license (within 30 days of hire if currently do not possess) and safe driving record required
- Certification in Targeted Case Management preferred but willingness to go through certification process required if not certified
