DescriptionLifeStream is a behavioral health and social services organization that provides high-quality treatment, education, care management, rehabilitation, child welfare, primary care and homeless services to children, adolescents, and adults. Located in Central Florida, LifeStream primarily serves the residents of Lake, Citrus and Sumter Counties, with additional programs located in Orange, Marion, Hernando countiesJOB SUMMARY : The Case Manager II functions as an advocate and support person to those individuals assigned to their assigned program and links them to appropriate services and monitors the progress or lack of progress by the individual and his or her family. The Case Manager II will work closely with the individual, the individual's family, LifeStream staff, and other community organizations to meet their needs and to allow the individual to live as independently as possible within the community.ESSENTIAL FUNCTIONS and RESPONSIBILITIES : The list of essential functions, as outlined herein, is intended to be representative of the tasks performed within this classification with or without an accommodation. Advocates in the client's best interestConduct a thorough assessment of the consumer (family's) home stability to establish significant changes in the consumer (family's) home environmentBased on this assessment, develop and review the service plan as required to meet consumer needs when there are significant changes in the consumer (family's) situation Schedule face-to-face contact with consumer (family) served, preferably in their natural living environment as required assigned by program guidelines;
