Job Description:
- Provide case management services to adults aged 18 and older with serious mental illness
- Link clients with needed services, such as medical appointments, court hearings, and other activities outlined in their Plan of Care
- Monitor clients' progress and provide individualized supports
- Assist clients with transportation to medical appointments and other activities
Requirements:
- Bachelor's degree with major coursework in human services field required or equivalent experience
- Targeted Case Manager certification required
- Minimum three years of prior experience working with individuals with serious mental illness
