Community Habilitation
Responsibilities
Under the direction of the Program Coordinator, provides services to parents and individuals in the Community Services Community Habilitation Program. For self-directed/co-managed services, receives training and instruction from the individual or designee who is co-managing the service.
Qualifications
Bachelor's degree in field of education, social work, psychology, rehabilitation, or related human service field from an accredited program and 2 years of experience serving individuals with developmental disabilities with a minimum of 2 additional years working as a case manager in the MH or DD field; or an Associate's degree in Human Services and 2 years of experience serving individuals with developmental disabilities and a minimum of 3 years' experience working as a case manager in the MH or DD field. An outstanding candidate with a combination of education and or experience working in the human service field may be considered. Must have a valid New York State Driver's License, a reliable vehicle; meet Lifetime Assistance Vehicle Operator Requirements and agency standards to transport service recipients
