Location: Jackson, Mi | Full-Time | Monday-Friday 8am-5pm

LifeWays is seeking a Case Manager to provide culturally competent, trauma-informed support to individuals in need. You’ll play a key role in helping consumers access essential services such as healthcare, housing, financial support, and more, all through a person-centered approach.
What You'll Do:

Develop & Implement Personalized Plans: Work with consumers to create an individualized plan of care, ensuring services meet their unique needs and goals.

Coordinate Care: Act as a liaison between consumers and service providers, advocating for continuous, high-quality care.

Support Independence & Recovery: Help consumers build natural supports, navigate transitions, and manage crises.

Monitor & Adjust Plans: Regularly assess progress and make adjustments to ensure consumers get the support they need.
What We’re Looking For:

Education: Bachelor’s degree in social work, psychology, or related field.

Skills: Excellent communication, advocacy, and problem-solving skills. Strong understanding of person-centered planning and community resources.
