LifeWays is looking for a Nursing Home Case Manager to join our team of compassionate, driven professionals. In this role, you’ll provide trauma-informed, culturally sensitive mental health support to individuals in nursing facilities and carry a small case load of community-based consumers. You’ll help people navigate systems, access services, and build a life of dignity, independence, and hope.
What You’ll Do
- Complete OBRA psychosocial assessments and ongoing mental health monitoring
- Develop and implement Individual Plans of Service (IPOS) using person-centered planning
- Coordinate care and connect individuals with financial, medical, and community supports
- Support transitions—whether from hospital to home, nursing facility to community, or school to work
- Provide crisis intervention and post-hospitalization planning
- Collaborate with a variety of providers to ensure holistic, consistent care
- Advocate fiercely for the people you serve
What We’re Looking For
- A degree in social work, nursing, psychology, or a related field
- Experience with case management and/or mental health services (preferred)
- Strong organizational and communication skills
- A passion for helping others live their best lives

Required Education/Certification/Licensure
- Bachelor's Degree in Social Work, Limited Licensed Bachelor Social Worker (LLBSW).
- Must be eligible to be credentialed as a QIDP and QMHP
Preferred Education/Certification/Licensure
- Fully Licensed Bachelor Social Worker (LBSW).
