LifeWorks of Austin, TX is looking for a Prevention Implementation Specialist to join our Navigation team! The Prevention Implementation Specialist provides daily implementation support and technical assistance to Trained Direct Service Partners. The Prevention Implementation Specialist provides supportive services to Youth who will receive flexible financial assistance through the Prevention Demonstration Project (PDP). This role provides Systems Navigation education and guidance to System Partners who are attempting to assist youth in accessing the appropriate part of the Youth Social Services Continuum. The Prevention Implementation Specialist ensures our System Partners know “the right door” for requesting services for youth they meet in various states of housing stability need. This position will regularly engage in independent and creative housing problem-solving to improve housing stability for referred clients of other agencies, while also being familiar with the various grant regulations and requirements that affect what options are allowable and available.
This full time, non-exempt position earns a competitive starting rate of $24.86-$27.17/hr depending upon experience. RESPONSIBILITIES
- Develops healthy working relationships with both Trained Service Providers and Referring Partners participating in the PDP Planning and Implementation process.
- Conducts community trainings and educational presentations for Trained Service Providers and Referring Partners on the Youth Homelessness Response System, and the Prevention Demonstration Project in particular.
- Develops and distributes systems access job aids, guides, and resource reference materials for Trained Service Providers and Referring Partners.
- Completes Program intakes for youth being referred to LifeWorks for Flexible Financial Assistance by Trained Service Providers, including intake documentation, applicable program consents, and data entry.
- Serves as the primary point of contact and coordination for individuals in the process of being referred to LifeWorks for Flexible Financial Assistance.
- Affirmatively advertises and connects PDP program participants to appropriate and available supplemental support programs within Lifeworks, such as Peer Drop-in, Workforce Development, and Education.
- Affirmatively advertises and connects individuals experiencing homelessness (including PDP participants who enter homelessness despite the program’s assistance) to the Coordinated Entry System access points for permanent housing resources.
- Closely collaborates with Internal LifeWorks teams responsible for contract compliance, finance procedures, HMIS support, and future PDP-adjacent grants to identify areas for internal efficiencies and workflow improvements.
- Closely collaborates with LifeWorks’ Research and Evaluation Team to design, implement, and re-evaluate program monitoring measures such as qualitative interviews, surveys, and HMIS Homelessness Response System contacts.
- Compiles daily data entry of referrals and authorization forms into Customer Relations Management database; regularly tracks and enters expense reports and purchase requests into agency financial system.
- Participates in internal and external auditing and monitoring.
- Regularly interacts with agency staff, volunteers, donors, board members and/or client populations in a professional and confidential manner.
QUALIFICATIONS
- Requires a Bachelor’s degree in Social Work, Counseling, or related field and (3) years’ experience; or
- Equivalent combination of education and experience.
- Preferred experience with HMIS.
- Preferred experience with D365, including PowerBI.
- Proficient level of experience with Microsoft Office, including Word, Excel, and Outlook.
