Lifeworks is seeking a full-time exempt CFSS Service Coordinator to provide case management for Lifeworks Financial Management Services clients and families . The CFSS Service Coordinator will support individuals with disabilities and their families by managing their fiscal budgets, utilizing their funding to meet their needs and life priorities, and maintaining knowledge and understanding of regulations and Minnesota waiver programs. The CFSS Service Coordinator works as part of a greater Self-Directed Services team holding a lot of autonomy when managing their caseload.

Position Title : Community First Services and Supports (CFSS)

Department : Self-Directed and In-Home Services

Hours : Full-time 40hours / week, Monday - Friday (Exempt)

Location : The role is hybrid - 20% in-office

Compensation : $49,500 - $55,500

Bonus Eligible : Yearly

  • Thorough understanding of disability services and funding sources available in Minnesota highly preferred.
  • Travel in the greater metro area when necessary is required.
  • Must pass a background study with the Department of Human Services, a drug test, clean driving record and have reliable transportation to use for work driving purposes (e.g., in-office, on-site training locations, etc.).
  • Requirements

  • Four-year degree in human services or related field with one year for direct care to individuals with disabilities or individuals of 65 years of age or older or a Two-year degree in human services or related field with two years of direct care to individuals with disabilities or individuals of 65 years of age or older.
  • Demonstrated interpersonal skills that quickly build rapport with customers, county social workers, and administrative staff.
  • Case management experience related to self-directed services preferred.
  • Candidate must complete and pass a background study, pre-employment drug screen; have a valid driver's license free of violation; proof of motor vehicle insurance and reliable transportation for work purposes.
