The Entitlements Case Manager is responsible for supporting individuals in obtaining the public benefits and entitlements they are eligible for, including but not limited to food assistance, housing subsidies, healthcare, unemployment, disability, and other government-funded programs. This role involves conducting eligibility assessments, assisting with application processes, and providing ongoing case management to ensure clients receive the services and support they need to improve their quality of life.
Key Responsibilities:
- Client Assessment and Eligibility Determination:
- Conduct intake interviews and assessments with clients to determine their eligibility for public assistance programs.
- Review clients' financial, medical, and personal information to assess needs and eligibility for entitlements.
- Interpret eligibility criteria and ensure all necessary documentation is collected and submitted for processing.
- Application Assistance:
- Assist clients in completing applications for various public benefits, such as Medicaid, Supplemental Nutrition Assistance Program (SNAP), Temporary Assistance for Needy Families (TANF), and more.
- Provide guidance and support throughout the application process, including navigating online portals and submitting required paperwork.
- Case Management:
- Develop and implement individualized case plans based on clients' needs and goals.
- Provide ongoing support and follow-up with clients, ensuring that their applications are processed, and their entitlements are maintained or renewed.
- Monitor clients' progress and address any barriers or challenges they may encounter in accessing benefits.
- Advocacy:
- Advocate on behalf of clients in dealings with government agencies, ensuring their rights are upheld and their applications are processed efficiently.
- Assist clients in resolving issues or disputes related to denied or delayed benefits.
- Referral and Resource Coordination:
- Connect clients with additional services and resources, such as housing assistance, mental health services, and job training programs.
- Coordinate with other social service agencies to provide holistic support to clients in need.
- Record Keeping and Reporting:
- Maintain accurate and up-to-date case files and records of client interactions, progress, and documentation submitted.
- Prepare reports on client outcomes, including tracking the approval or denial of applications and the resolution of issues.
- Compliance:
- Ensure adherence to all relevant federal, state, and local regulations governing entitlement programs.
- Stay current with changes in entitlements and benefits policies, and update clients as needed on policy changes that affect their eligibility or entitlements.
- Collaboration:
- Work collaboratively with other case managers, social workers, healthcare providers, and community organizations to meet clients' needs.
- Participate in team meetings and case conferences to discuss complex cases and best practices.
- Discharge Planning:
- Create a comprehensive plan tailored to the individual, outlining clear steps and goals for independent living within a 12-month period.
- Ensure eligibility for financial assistance, healthcare coverage, housing programs, and other benefits is established and in place before discharge.
- Facilitate access to community services (e.g., vocational training, mental health support, housing assistance) that will support the individual’s independence.
- Set realistic milestones and a timeline to measure progress toward full independence, with the goal of achieving this within 12 months.
- Track the individual’s progress throughout the year, offering periodic check-ins and adjustments to the discharge plan as needed.
Qualifications
- Associate degree or bachelor's Degree in Social Work, Human Services, Psychology, Sociology, or a related field
- Proven experience in case management and discharge planning
- Strong skills in crisis intervention and management
- Proficiency in addiction counseling and behavioral health
- Familiarity with motivational interviewing techniques
- Experience in supervising and mentoring staff
- Knowledge of child welfare and Medicare systems
- Understanding of workforce development and Section 8 housing programs
