- Will possess a High School diploma or GED with two (2) years’ experience working with troubled child as a para-professional, volunteer or intern in a social service, residential, educational or institutional setting or (2) years college credit (30 credits) and (1) year experience with troubled child.Bilingual in Spanish preferred but not required. Must be 21 years of age Must be able to read, write, and communicate with co-workers, medical personnel, and other person’s necessary to care for the child’s needs. Must have computer skills in Microsoft office. Must be physically, mentally, and emotionally capable of performing assigned task and have the skills necessary to perform assigned task. Must demonstrate competency, prudent judgment, and self-control in the presence of children and when performing assigned responsibilities; must possess excellent organizational, writing, analytical, and processing skills. Should be able to perform conflict resolution or crisis intervention and have excellent communication skills. Must be able to embrace cultural diversity awareness. Must have the ability to work as a direct care staff with a diverse population. Must have a valid state driver’s license. Must be able to pass drug screening and background checksRESPONSIBILITIES: Serve as a positive adult mentor for all child by respecting their boundaries, privacy, and confidentiality. Responsible for supervising, transporting, supporting, and encouraging assigned child. Responsible for daily documentation, incident reports as necessary, and the distribution of medication for assigned child. Assist child with all daily living activities, homework, etc. Provide child care services for each assigned child; Being aware of and accountable for each child’s on-going activity; Providing the level of supervision necessary to ensure each child’s safety and well-being, including auditory and/or visual awareness of each child’s ongoing activity as appropriate; and being able to intervene when necessary to ensure each child’s safety. Be aware of the children’s habits, interests, and any special needs; Provide a safe environment; Cultivate developmentally appropriate independence in children through planned but flexible program activities; Positively reinforce children’s efforts and accomplishments; Ensure continuity of care for children by sharing with incoming staff information about each child’s activities during the previous shift and any verbal or written information or instructions given by other professionals; Serve as a positive role model; Deep cleaning, laundry and preparing breakfast, Lunch and, or Dinner Keep Facility clean at all times by assisting child with chores Maintain client progress notes and assist in developing goal-oriented service plans; Provide client supervision and maintain appropriate documentation; Establish helping relationships with client by demonstrating the ability to apply clinnically directed concepts and interventions; Report all inadequacies in provision of service to clients as specified by Royal Heritage and contract guidelines; Prepare and submit well-organized documentation and reports within required timelines demonstrating ability to report facts and apply policies and procedures; Frequent bed checks when working night shift Staying awake all night while on shift Keep LCCA and Team leader informed of all observations made; Provide crisis intervention for clients consistent with House of Hopepolicies and procedures; Attend and participate in training, workshops and in-services provided by Royal Heritage. Maintain confidentiality in all areas of client and program operations; Maintain professional and ethical standards and professional demeanor at all times; Comply with the agency and program and policy procedure manual; and Comply with all Licensing Standards Other duties as assigned by Team Leader that do not interfere with direct childcare responsibilities.DIRECT CARE STAFF $12 to 13.40 per hour[3/13, 1:10 PM] Oluwabusuyi Don: Essential Functions:Must ensure that youth are properly enrolled into the program with all needed information during the intake process.Conduct initial intake interviews of youth to include gathering of familial information and to establish rapport with the youth.Must perform monthly audits of the program’s active case files (physical and on RH’s web-based system) and maintain appropriate documentation to ensure compliance with RH, State, and Federal guidelines.Review all assessments, and any other documentation or assessments for accuracy of information while ensuring proper spelling and grammar is utilized prior to case manager submission to any entity.Serve as the main point of contact for approving initial placements, youth being transferred and for requests for information in coordination with LCCA.Ensure that all physical and electronic case files are closed upon youth leaving the facility in accordance with minimum standards.Maintain a daily updated list of the status for all cases (census) to ensure that adequate progress is being made.Ensure that travel arrangements are completed for program staff transporting youthEnsure the timely completion of the initial Individual Service Plan due within 72 hours of the youth’s arrival to the facility in accordance with RH, State and Federal requirements.Document all actions taken and contacts with youth, sponsor (contract holders), and stakeholders (caseworker, CASA, lawyer, etc.) in the form of progress notes (efforts) as required by RH, State and Federal contracts.Ensure the provision of two weekly telephonic contacts with family of origin, primary caregiver and/or sponsor.Facilitate attorney to client contact as requested by youth.Ensure that Treatment Team meetings are conducted on a weekly basis with representatives from all departments at the facility.Other Functions:Attend all organizational required trainings and facilitate trainings as needed.Attend all departmental and program meetings to ensure that up-to-date information is received and/or provided and to ensure that changes to policy or practice are implemented and adhered to.May serve as the Administrator In Charge of the facility’s overall operation in the absence of the LCCA.Create, identify and attend trainings that will enhance professional growth in the area of case management or supervision.Assist with the submission of monthly RH reports, funder’s quarterly reports, and end of year reports.Provide and facilitate information as needed for RH and/or funders monitoring activities.Must assist in the evacuation of youth as needed due to inclement weather conditions, natural disasters, or other unforeseen occurrences.Conduct monthly supervision meetings with all direct care and maintain an ongoing record of the outcomes of the meetings conducted in coordination with LCCA.Attend training that will enhance professional growth in the area of case management, management and documentation or other topics as deemed appropriate by the LCCA.Maintain a safe, clean and hazard-free work area.Ensure the proper supervision of youth at all times.Able to react to change productively and handle other essential tasks as assignedCASE MANAGER 45k to 50k p/aReplyReply allForwardAdd reaction
