ESSENTIAL FUNCTIONS
- Case Managers will develop and monitor the Person-Centered Plan process. This includes but is not limited to scheduling, meeting facilitation, team coordination, and plan development.
- Case Managers will complete progress notes/documentation by the deadlines set forth by MaineCare regulations.
- Case Managers will meet all billable productivity standards set by Lighthouse Community Care LLC.
- Case Manager will ensure timely and accurate completion of all comprehensive assessments, quarterly reviews, and other MaineCare related procedures.
- Case Manager will provide linkage to services to meet identified barriers through referral, advocacy, and monitoring of services in accordance with MaineCare policies.
- Case Manager will provide support to members to access needed resources.
- Case Manager will provide advocacy for members, prompt self-advocacy, assist them with making choices, achieving identified goals, and promoting community inclusion.
- Case Manager will maintain members records in the electronic medical record.
- Case Manager is responsible for their own schedule and will ensure all members are contacted and in accordance with State of Maine statues, and that sufficient time is allocated for follow-up activities.
- Case Manager will assist members in accessing and maintaining needed resources on an as-needed basis, including but not limited to funding sources for services, food supplement benefits, healthcare, housing, transportation, education, and vocational services.
JOB REQUIREMENTS
- Bachelors Degree in Social Work or closely related field: Psychology, Mental Health and Human Services, Behavioral Health, Behavioral Sciences, Social Work, Human Development, Special Education, Counseling, Rehabilitation, Sociology, or Nursing
- 1-year paid or volunteer experience in the field of Social Work or closely related field listed above
- Valid driver’s license with clean driving record and insured vehicle
- Excellent verbal and written communication skills
- Critical thinking skills
- Ability to work independently under limited supervision
- Strong professional boundaries

Pay: $21.00 - $22.50 per hour
