Description

Job Description Summary:

The LVN for Health Services and Programs is responsible for providing, monitoring and managing direct health and well-being, safety and quality care for clients receiving Health Services and Adult and Youth programs. They are also responsible for operating within the regulatory licensing guidelines of the State and organizational policies. 

The LVN’s role includes indirect supervision of the Youth and Adult staff to ensure the clients receive the highest quality of care and follow the state and regulatory licensing requirements and organization care expectations.  In collaboration with Health Services Optometrists and Program Management, the LVN will assist with training associated with regulatory and care requirements under the RN’s guidance and recommendations.

  • Manages regulatory licensing compliance for Health Services and Youth and Adult programs by ensuring facilities are maintained and in strict compliance with all state/federal guidelines and license requirements and company policy and procedures.
  • Investigate all incidents, medical emergencies and alleged abuse investigations and provide appropriate reports to required agencies.
  • Assesses and recommends continuous improvement to program policies and procedures to ensure they are safe, timely, effective, efficient, equitable, client centered, and compliance expectations are met.
  • Participates in quality improvement efforts to monitor the care provided to clients and identify opportunities for improvements and implement Plan of Actions (POA) to correct any issues or concerns.
  • Assists in training for all staff and volunteers in the least restrictive techniques, behavior management, active treatment, client rights, prevention of abuse/neglect documentation/data collection, emergency procedures, medication administration and other areas as needed.
  • Ensures that all staff and volunteers are trained in accordance with each client’s individualized care plan and individualized needs.
  • Develops, monitors and documents individual client behavior, medical emergency information, medication administration, vital signs, care plans, progress notes, health assessments and physician orders.
  • Adheres to physician orders and dietary plans for clients.
  • Coordinates nursing services for clients to ensure continuity of care.
  • Checks and ensures all medicine cabinets are locked and responsible for documented reports as needed.  Responsible for proper disposal of medication and medical supplies.
  • Ensures that all medication and required first aid medical supplies are stocked and available.
  • Assists with planning and coordinating health care events and flu shot clinics.
  • Attends team meetings, Leadership/Management meetings and other meetings as needed.
  • Performs other duties as assigned.

Qualifications


Education, Licensure and Experience:

  • Must have a LVN license and in good standing in the state of Texas
  • Bachelor's Degree in Nursing, Social Work, Human Services, or another related field is required or Associate Degree in a field related to human services.
  • Must have  2 to 3 years of related licensed vocational nurse experience.
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to long-term care.
  • Relevant experience in education, social work, non-profit or a related field.
  • CPR and First Aid Certified.
  • TB test annually and administer TB tests to Direct Care and other staff
  • Maintain CEU’s and required trainings mandated by HHSC (Health and Human Services Commission)

  • Ability to prioritize and work at an elevated level of accuracy.

  • Ability to work independently or in a team setting.
  • Ability to lead and guide staff to meet the customer and client expectations.

    Supervisory Responsibilities:

  • This position has no supervisory responsibilities and leads and guides staff to meet departmental goals and state licensing compliance. This position also interacts and collaborates with other leaders and managers in sharing ideas and information to Identify and recommend organizational improvement.
