RN Home Health Case Manager - 16 week

- Day shift - 8:00am - 4:30pm, Monday - Friday w/rotating weekends

- Required: VT RN License or compact, 2 years Med/surg or Home health care experience
The RN Case Manager:
- Develops a care plan in collaboration with the patient's physician, the patient, and their family, or the primary caregivers upon admission. Periodically reviews the patient care plan and recertifies the patient care plan as appropriate.
- Coordinate the patient's care plan through interdisciplinary communication and case conferences, as appropriate. Advise other disciplines of discharges in a timely manner.
- Provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals. Communicate significant changes to the patient's physician and VNA colleagues.
- Document evaluations, treatment goals, and objectives.
- Teach the patient and caregiver self-care techniques as appropriate. Provide medication, diet, and other instructions as ordered by the physician and utilizes opportunities for health counseling with patients and caregivers.

Duties
- Conduct comprehensive patient assessments to determine health status and needs.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Administer medications and treatments as prescribed by physicians.
- Monitor patient progress and adjust care plans as necessary.
- Provide education to patients and families regarding health management and disease prevention.
- Collaborate with interdisciplinary teams including physicians, therapists, and social workers to coordinate care.
- Maintain accurate documentation of patient interactions, assessments, and care provided.
- Ensure compliance with all regulatory requirements and best practices in home health care.
Experience
- Current Registered Nurse (RN) license in good standing.
- Experience in a Level I Trauma Center or similar high-acuity environment is preferred.
- Knowledge of geriatrics and physiology is essential for effective patient care.
- Familiarity with managed care systems and medical-surgical nursing practices is advantageous.
- Previous experience in outpatient clinic settings or operating rooms (OR) is a plus.
- Strong patient service skills with the ability to communicate effectively with diverse populations.
