Job Summary
We are seeking a dedicated and compassionate Registered Nurse to join our healthcare team. The ideal candidate will possess a strong background in patient care, particularly in geriatrics and senior care settings. This role involves providing high-quality nursing care to patients, administering medications, and collaborating with healthcare professionals to ensure optimal patient outcomes. The Registered Nurse will also be responsible for assessing patient needs, developing care plans, and educating patients and their families about health management.
Duties
- Conduct comprehensive assessments of patients’ health status and needs.
- Develop, implement, and evaluate individualized care plans based on patient assessments.
- Administer medications and treatments as prescribed by physicians.
- Provide skilled nursing care including catheter care, injections, and memory care support.
- Monitor patients for changes in condition and report findings to the healthcare team.
- Educate patients and families on disease management, medication administration, and health maintenance.
- Collaborate with interdisciplinary teams to coordinate patient care effectively.
- Maintain accurate documentation of patient assessments, interventions, and outcomes.
- Stay informed about advancements in nursing practices related to geriatrics and senior care.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Strong knowledge of physiology and geriatric nursing principles.
- Experience with DME (Durable Medical Equipment) is preferred.
- Familiarity with MDS (Minimum Data Set) assessments is a plus.
- Experience administering injections and providing catheter care.
- Knowledge of dermal fillers is advantageous but not required.
- Excellent communication skills with the ability to educate patients effectively.
- Compassionate demeanor with a strong commitment to patient-centered care.
- Ability to work collaboratively within a healthcare team environment.
