Job Title : Weekend HospiceOn Call Registered Nurse
Job Summary : Primary functions are to administer nursing care for patients of all ages in their place of residence after hours and / or on weekends. The On Call Registered Nurse will be responsible for responding to urgent patient and family needs, managing symptom control, and providing support during end-of-life care. This role requires a timely response to calls, effective clinical assessment, and the ability to work independently.
Job Qualifications :
Education : Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Licensure : Current state license as a registered nurse (RN) and / or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact. Current driver's license.
Experience : Experience as a registered nurse in a clinical care setting. Hospice experience preferred.
Skills : Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Specialized skills preferred in palliation and end-of-life. Must read, write, and comprehend English.
Transportation : Reliable transportation and valid auto liability insurance.
Environmental and Working Conditions :
Works in patients' homes and facilities in various conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather.
Essential Functions :
- Provide on-call hospice nursing care in response to patient or family needs during assigned shift.
- Perform patient assessments, manage symptoms, and administer medications as needed.
- Collaborate with the interdisciplinary team (physicians, social workers, chaplains, hospice aides, etc.) to ensure comprehensive care.
- Consults with and educate the patient and family regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs.
- Maintain accurate and timely documentation of care provided using EMR.
- Communicate changes in patient condition to the care team and recommend adjustments to the care plan.
- Provide emotional support and guidance to patients, families, and caregivers.
- Ensure that all interventions adhere to hospice philosophy, polices, and regulatory guidelines.
- Maintain timely response to on-call duties, ensuring patient and family needs are met in a timely manner.
- Triage duties may be required for some employees in this role, depending on specific assignments and departmental needs.
- Applies concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.
- Informs physician and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient and family response to care.
- Demonstrates commitment and professional growth and competency by participating in the Hospice sponsored in-service programs and maintaining or improving competency.
- Promotes the Hospice's philosophy and administrative policies.
- Provides effective communication to patients, family, team members, and other health care professionals.
- Perform emergency admissions of new hospice patients as needed.

Requirements
Physical and Mental Effort :
Prolonged standing and walking required, with ability to lift up to 50 lbs. and move patients. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient and family individualized psychosocial needs. Requires hand-eye coordination and manual dexterity.
